Not sure on 1) - we'd need to look at the difference between M and G on that segment. There is some possible evidence CO "modifies" the YB or M fare at the last minute to drop the buy-up cost.

On item 2), you should have said something to the GA. I wouldn't have hesitated to speak up and tell the GA that Elites were still on the upgrade list and this would be reported. Please post the date and flight number here so CO lurkers can do a check on their own, and then open a case with Customer Care and report it.

If the pilot's contract terms required a seat in F, he would have been assigned one as positive space. If the action was permitted for some reason, then CO can close the case....but unless this stuff gets reported, rogue agents will still flaunt the rules.
