I must say that this is the complete opposite of my experience. I have always managed to get an appointment at JFK (probably due to the fact that I normally get the first or second evening flight out) and have always found the staff really friendly!
At LHR it is a lottery (albeit with slightly better chances than 14 million to 1!)and what makes it worse is that there seem to be more booths than staff so it's not that they don't have the space but that they don't have the therapists. I'd say that I'd probably get a treatment about half the time, and normally arrive about 2.5 hours before the flight. Maybe it is down to the time I normally fly - mid-afternoon - so the morning rush has gone and the evening rush has yet to get started.
I don't really agree with
Swanhunter - although I wouldn't pay for the treatments they are OK
