I once booked a RT about 4-5 weeks before trip; then I forgot that I had booked and a couple of weeks before the trip, I booked the exact same flights and days again. I didn't realize this until both upgrades cleared on the outbound and I got two email alerts (I didn't know this was even possible). I called CS and they fixed it at no cost, even though it was 100% my fault.
