I have flown in that seat on multiple occasions on the 747s and A340s.
It is definitely NOT flat - the first issue is the angle being less than 180 (above the waist only) and the second issue is the "headrest" which protrudes up.
I even tried sleeping "backwards" with my head on the ottoman (covered by a blanket), and in this position you can certainly tell that the seat is not flat.
