I had a card that refused to scan and after being told that it would cost $25 to send me out a new card, I politely declined and hung up and like Vulcan, I refused to pay money to make CO employees lives easier, so I would inform the PClub agent that I had a faulty card and he/she would have to manually enter the number.

After a while (I think it was the SFO club) the agent instead of just entering the number and returning the card to me, picked up the phone and called the OPSC and spoke with somebody for a minutes stated that my card was faulty and asked that they send me out a new card. Apparently if an agent from a club calls there is/was no charge for a replacement card.
