Is it inconvenient to get out of middle CW seat?

I have never flown CW, but I have booked a reward booking for my Grandparents who are flying to LHR - YYZ on a 747. My Gran has limited mobility but does not need to use any walking aids (she has difficulty in bending her knees). I had originally wanted to book 20EF but have noticed that they are not available to pre-book. I appreciate that it is possible for these seats to open up when OLCI opens (or not).
I want to book EF for both of them as I feel they would me most comfortable in those seats as they will be seated next to each other as opposed to apart. However, considering my Gran's limited mobility, I was wondering if I was not able to book 20EF, but was able to book EF in another row, how difficult would it be for her to get to the aisle when she needs to use the loo (which will be more frequent than the 'average' traveler)? My reason for asking is that I get the impression that if the person, on either side of EF, decided to put the foot rest down, that she would need to step over the foot rest in order to get to the aisle which I envisage would be difficult if not impossible for her.
I appreciate that if 20EF is not available and if you guys tell me that it would be difficult for her to get around the foot rest issue then I will book an aisle seat for her when OLCI opens.
