as for sfo to jfk, there's always the chance that you can get a change in equipment to a non BE configured 757, which would be a downgrade, or it can also go the other way and you can wind up with a 767-300ER. This has happened to me.
However, for the flight to Italy, you'll have no worries as the worst that can happen is that flight would get subbed by a 764 with lie-flats.
