OP had sent in a missing mileage request and had the flight denied due to being "free."

I don't however believe the e-cert had much, if anything, to do with it. The denial for "free" flights can happen for many reasons--I've had upgraded flights denied for that reason before, for example, as well as partner flights (US, CO).

Always, a brief note/letter plus the BPs faxed to MP has resolved the problem. Well, except for when UA and DL had codesharing (!) and one of my DL flights never posted to UA.
