Visa applications need to be submitted to the country that is the intended visitor's main destination. If there is no main destination then the visa application should be submitted to the first point of entry.

So - if someone is visiting Country A and changing planes in Country B on the way to Country A then it's country A that needs to issue the visa.
And once the visa has been issued, there is no problem visiting any Schengen country. I know people who have multiple-entry Schengen visas issued by country X, and are regularly visiting country Y without any problem.
