If your Amex card has free, "secondary" loss/damage coverage, that's all you need to take care of the rental car itself, as long as:
1. you do not rent in one of the handful of countries that Amex excludes from coverage; and
2. you do not use a free-day or dollars-off coupon on the rental, which would void the free Amex coverage; and
3. you do not rent a car type that Amex excludes from coverage.
As for third-party liability, in some foreign countries, it might be included with all car rentals; in others, you would have to purchase SLI coverage.
If you plan to rent a car
in the U.S. for at least ten days a year, a non-owned automobile liability policy may be more economical than buying SLI for every U.S. rental. And in NYS, I believe that such policies also cover loss/damage to a car rented in the U.S.
