I know a friend did it this way a while back.

Download the application form for applying their card.
- Fill out the information, obviously
- If you already have their card, then attach your card.

Attach a letter requesting for status match.
- Which obviously you will need to write something similar of you will fly with them if you get matched...

Attach your past 2 or 3 years traveling pattern from 1 specific airline.
- Attach your current membership card from that specific airline.

E-Mail them.
- The problem is I cannot find the specific e-mail he sent. It's not by fax or sent to a customer relations desk. It's sent to a specific person working for memberships in CI. I guess you can send them fax to see what will happen?
