I was in Victoria, BC, and took the ship to Seattle for a few days, so I guess mine counted as a "new" visit whereas perhaps your previous entry meant that there was no need for the form?

Upon returning to Canada, the immigration official didn't take my I-94W away (I suppose I should've proactively given it to them) but thankfully the BA F check-in at YVR were able to take it off me!
