If you do not have medivac you will have to do with local care until you are fit for travel by normal means. With Medivac you will be taken to the closest location where care equivalent to you home country is available. (copied from my parent's policy).

MIL had same policy, three years ago she develop pain which eventually was discovered as some kind of gallstone infection and complication. Because she had the medivac coverage she was airlifted by private aircraft with nursing care along with one travel companion from Outer Mongolia to Hong Kong where she was operated on.

Base on medivac cost and what my extended family has paid in insurance coverage I would say we are pretty far ahead in the game.
