1. You cannot get tickets issued anywhere other than the country of origin of the first flight & tickets are charged at the rate for that country.
One exception applies - you can have AA, for example, reserve the ticket/flights for you and give you the locator reference. You then arrive in the city of departure (in your example FCO) and confirm/pay for the ticket at the AA desk there & at the Italian rate.
