..And how do I actually go about buying the ticket ? Once I've decided the routing, can I call BA (for example) and confirm the ticketing, then just turn up at the ticket counter at CAI and they issue the ticket there ? Or some other way ?

Best way is to get BA in UK to book the entire reservation first. THEN call BA in Egypt to say that you'd like to collect and pay for the ticket at CAI airport at this time... and to please set up the proper ticketing arrangements so that there will be a minimum wait for you. They should then queue the message to London to fare quotes.

In theory, you could just ask UK to do that.. but in many times, I have heard that the BA staff are too dense to realise the difference between issuing in London and issuing elsewhere. But give them a try anyway... after the booking, tell them to queue the reservations to their rate desk and say that you'd like to pay and issue the ticket in CAIRO.

If they give you a hard time, then opt for the second option.
