I have travelled with hand luggage only on the first leg of the trip, and checked in hold-luggage for the second leg of the same PNR so I know that it's definitely possible to add luggage part way through.

I haven't first hand experience and therefore can't answer the question about whether you'd have to pick your bag up from the first leg, but I can't see why you'd have to.
