A few points:
1) Israeli consumer laws allow (irrespective of the rules of the ticket) to cancel a ticket purchased online or on the phone within 14 days from the date of purchase and at least 7 working days prior to the date of travel for a charge of 100 shekels (about $28) or 5% of the ticket price, whichever is lower.
2) The $20 and $45 change fees are just admin fees for making changes over the phone rather than online and would be in addition to whatever fees are attached to the class of the ticket.
3) The OPs example is indeed strange. Normally fully flexible is just that with no charges for changes or cancellation (except for the charges I noted in 2 above if done on the phone).
