Question UA flight number, US metal--what to request for missing mileage credit?
My wife bought and flew a UA ticket for which her two US metal legs (using UA flight numbers) haven't credited to her account. If I understand the somewhat unclear instructions at the UA site, the form she should fill in to retroactively claim credit should list US as the carrier and provide the US flight number. Is that correct?

Did a search on this and came up empty.
