If you still have your boarding pass stub, make sure your number is printed on it following "FQTV". If that's blank, you won't get credit unless you call them.
Boarding passes have my number, just a lot more leading zeros than the number on my Frontier account (I think this is due to Frontier using more digits in their frequent flyer numbers than Midwest did).
