No (some CO EX operate out of Terminal C)
Yes. (CO EX may, but usually does not, operate out of terminal C).
Yes. (There is a shuttle between the CO part of Terminal A and all C concourses, which are all connected inside security.)

Even flightaware is confused about this (arriving at a C-terminal gate in terminal A).
