The rule exception allows a second entry and exit of Europe provided one is transit between one of the named countries and another continent.
So your two transits of LHR (one from NBO and the other to NBO) are both within the rules. (By the way only one of them needs to be a transit)
However, there is no exception allowing a second entry and exit of Africa - so your xLHR-JNB-HKG... is not allowed
