It's a while since I've done one, because I avoid BA wherever possible, but the ones I was given were A3 folded in half (so 4 x A4 pages) and the seal was along one side only so it would be easy enough to peer inside and see what someone had said.
The best method would be for questionnaires you complete at your leisure and post off, or for the EMailed web-based questionnaires post-flight to be expanded.
I've had too many bad experiences with BA - some of them with crew (or other staff) and some of them because of the actual product - to think that there is any sort of 'political will' for the company to genuinely want to improve themselves.
Time will tell whether the new regime, now Walsh has gone to IAG, will be able to reverse the decline.
