My understanding of the bassinet situation (from what I've read, not experienced): They are offered only in the bulkhead position. You may not be able to get pre-assigned seats in the bulkhead on some/many airlines as these are often held for airport assignment. Therefore, you may not be able to count on having the bassinet. Besides which, there are size/weight restrictions, which I suspect your 1.5 year old will have exceeded.
Yes, lap baby means that the baby is held in your lap throughout the flight, except if the bassinet happens to be available, you can lay them there. However, even if the bassinet is available and your child will fit based on size/weight, I would imagine it to be a very bad idea to leave said child ALONE in the bassinet since a 1.5 year old would be mobile.
If you are set on doing a lap baby + 1 seat for the other child, there is nothing saying that you can't swap which child is strapped into the car seat if you need to change the other one. However, if you are the one who has to pee- I suspect you will have to take one of the twins with you to the restrooms.
The logistics of doing this with toddler twins is simply mind-boggling to me, though, and you are braver than me!
