As for carding everyone, the result of being busted for underage drinking can result in the liquor license being pulled for the establishment. Think of that Chili's in ORD not being able to sell, its a huge loss of revenue.
At my job if you get busted you have to pay the fine yourself, and it will result in termination.
I don't know if by OTC you literally mean over the counter, but you can still get pseudoephedrine without a prescription which is what I consider OTC. It just happens to be stored behind the counter and you have to ask for it. Still requires an ID.
