I wasn't suggesting that Harley Street doctors are likely to take cash for a note either. I was only making the point that faced with a long journey in uncomfortable surroundings I had gotten stronger painkillers than normal but asking for a note would have been pointless (even though she was really rather nice) given we were banned from anything other than Y, even on trains.(and partly because of the post before mine)
