I wasn't suggesting that Harley Street doctors are likely to take cash for a note either. I was only making the point that faced with a long journey in uncomfortable surroundings I had gotten stronger painkillers than normal but asking for a note would have been pointless (even though she was really rather nice) given we were banned from anything other than Y, even on trains.
