As said, the hotel can add an extra person surcharge. If this is a non-US hotel you need to make sure that there are actually two double beds that can accomodate three adults. Many non-US hotels have rooms that will only allow a two person occupancy.

The breakfast or lounge access is for the Honors member and one guest. If they give you an extra one it is a bonus but not to be expected.
