Something manually inserted into an orifice can also be manually removed ... either by the terrorist, or by security officers (voluntarily or involuntarily).
From the perspective of a terrorist
organization, surgically implanting the explosive device makes it more likely that the device will actually make it to its desired destination.
Disclaimer: I'm not saying that this is a likely threat ... just that the mode of conveyance does have some advantages. (It also has plenty of disadvantages.)
