I have seen GAs in a hurry who don't pay much attention when the scanner starts beeping from an invalid boarding pass. In fact, I've gotten nailed on a few of these where my BP was invalid for some reason, the scanner started beeping, and the GA just told me to hurry up and get on board. It's taken a long time for the miles to post from such segments.
