Are there metrocard dispensers in the terminals? That would seem to make a lot of sense, with more prominent signage that passengers need to purchase a metrocard, or have exact change, prior to boarding.
The other thing that often creates delays is tourists getting on and having lots of questions for the driver. Having questions is understandable; standing so as to block all other passengers from boarding is not. Move the f*** out of the way while asking your questions!

This would be awesome, but again I question how practical it is -- that is, how much demand there is for it and how they could schedule the buses in a useful way. I ride the M60 and the Q33 a lot, and I haven't yet found a predictable pattern for when the buses are going to fill up with Manhattan-bound passengers. The buses often fill up during airport employee shift changes, but many of those passengers disembark in Queens, so wouldn't be interested in an express bus. I can't imagine there's enough demand to run them with great frequency. And if they only run them once an hour, then what's the point. Of course, it's possible that if they begin the service, the demand will follow, but I suspect the MTA's logic is that the benefits of the service (saving Manhattan-bound passengers maybe 10 minutes) are far outweighed by the cost.
