I read a thread at least four pages long on another web forum of parents planning to boycott AA over this.
Many of them do have strollers over 20 pounds, and they are angry.
The flip side of it is the impact it has overall on turn times etc. On a side note I was in a shop a few weeks ago and kept having to backtrack down aisles because a woman with a stroller filled the entire aisle and there was no way to pass on either side of her. That stroller was definitely larger than a wheelchair, for instance.
(Edited to add that the thread I referenced is now 6 pages and they are crying discrimination) And Eclipsepearl is correct - someone complained that their stroller was under 20 pounds but that they were not permitted to gate check it due to the size.
It may not be an issue on many routes, but on leisure-heavy and child-heavy routes such as flights to MCO I can see this as an issue. I am always amazed at the gate turn times outside the US vs flights in the US, and gate checked strollers do add to the issue (as do oversized and number of cabin baggage)
