I guess I understand this -- once a passenger is served food or drink, no part of that food or drink goes back into service. Probably a good health rule. It was a bit weird though because the cup of ice was only on my tray for about five seconds, more or less within her view the entire time.

Doesn't matter. Once the ice leaves her bucket it is considered dead. Health regulations prevent her from putting it back in the container.
Same thing if she gives you a whole can of coke. You pour yourself one glass only wanted one glass, the can has to then be tossed....... Always better safe than sorry :-)
