Not true - even near misses ought to be recorded. This way, if there's a trend of people nearly having accidents, then the problem can be sorted before someone actually does.

(health and safety departments often moan that they don't get enough accident forms for minor accidents, because people don't consider them serious enough - but it just ends up skewing their statistics.)
