On the upper right-hand side of United.com
click on the Customer service link.
Next, click on the Contact United tab.
In the Post-travel support section, click on the Customer Relations link.
From the drop down menu on the page that loads, select Mileage Plus.
The page should change to an Email Mileage Plus form.
