Aside from the shocking state of the food, the big mistake IMO was how they split the (minimum?) crew around the plane.

There were 11 FAs and 274 passengers, including a full F cabin. Why on earth you stick two very junior crew up front without any support is a mystery to me, but was presumably instrumental in ensuring the service was so slow.
