I finally figured it out myself after carefully reviewing my Chase monthly statements and a thread on FT about the bizarre rules used to post EQMs (e.g., only UA purchases from Rosemont, IL count and EQMs post only after 4 weeks had passed AFTER the end of the billing cycle).
