I received (unasked) a 5-star card yesterday, so I guess they are doing it automatically. I'm fairly rate-conscious on most of my rentals, and tend to use Avis (which also gives 500 BA miles per rental) or National a lot. Probably if I consolidated all my rentals with Hertz, I could make it to President's, but I'm not convinced the benefits are worth the extra cost.
