ANA Family Account (Pooling of Miles for members residing outside of Japan)

Question 1)
Website: "Upon registration of such a family Member, documentation must be presented of his or her relationship to the Primary Member and of his or her address outside of Japan."
When I register online for a family account, do I really also have to send something in? I did not see that in the online regristration process. Can someone who uses family accounts please share their experience? Did you send documents in after applying online?

Question 2)
Website: "AFA allows you to combine the miles earned by your family, and to redeem awards such as ANA and Partner Flight Awards, and ANA International Upgrade Awards."

a) Would a miles redemption flying LX (Swiss, a star member) and no other airline be considered a partner award? If you redeem on a single star alliane carrier, is it a Star award or partner award?

b) With an AFA account set up, will the Primary Member have access to the pooled miles online? Or only by phone?
