Next trip is with Dollar @ IAD through www.usrentacar.co.uk and itincludes "LDW, SLI, UMP, Taxes and Fees". And at <£500 for 3 weeks' rental of an "Intermediate SUV", I think that's a good deal.

YMMV of course, especially at high-density tourist destinations.
