What do they do these days with orthotics/orthopedic shoes?

The TSA website still says things like:

Security Officers should not be asking you to remove your orthopedic shoes, appliances, or medical device (insulin pump, feeding tube, ostomy or urine bag, or exterior component of cochlear implant) at any time during the screening process.
However not having flown since the sexual assault policies were implemented can anyone tell me what will happen when I refuse to remove my shoes and put them through the X-ray machine? I assume this will trigger a grope, which is fine since I would opt out anyway. But have they gotten more clued about the fact that some people experience severe pain if they stand or walk without their medical supports?
