KWL- That form is only good for Alaska/Horizon credit. According to the website sending an email is the way to do it for Partner requests.
I guess I'll let it go for another week or so then try calling. I'm just a litle surprised I didn't at least get a form letter acknowledging receipt of my request.
Also, as mentioned, I did get credit for the first of two DL segments that day, so that's why I was surprised I didn't just get get credit for both at the same time.

After you submit the form, an employee will email you back within a few days either saying she found the flight, or asking for a copy of the boarding pass. Sometimes those DL flights take a while to post, but since you kept the boarding pass, you should be good to go.
