I am ready to book a new award ticket with Aeroplan and the agent told me that no middle name required in the ticket as long it is properly recorded in the secure flight info, all will be fine!
What your agent told you reflects my experience flying to NYC with SAS.
Note also that Middle name is not asked in all US customs documents.

Finally what is weird is that Lufhtansa and SAS do not ask for exactly same informations before flight.

For people without visa eligible to US visa waiver program (mainly UE passports holders) don't forget to register on https://esta.cbp.dhs.gov and to pay the 14 dollars tax implemented by Obama administration. Register can be done just before taking the flight if you are not in one of the "sensitive" categories.
