preset (at seat) for the First cabin on N. America flights.
Additional pillows are however often stored in overhead bins throughout the cabin, and are free for any passenger to use. Ditto for blankets.
But you are correct that it is only guaranteed (so to speak) for First Class. I think some folks see them littered around in coach on some flights and the expectation then is that they are entitled to one on demand on subsequent flights.
Having pillows and blankets preset at every seat in F is a nice touch that I really appreciate on DL, along with the water bottle. My last flight on AA - seated in First Class nonetheless - when asking for a pillow, I was bluntly told that "we did away with them after 9/11, we can't afford to provide such things anymore".
