Has anyone ever had problems with a cancelled award ?

I cancelled and award a few days ago and the miles didn't show. I contacted bmi DC and they told me the award was already cancelled long time ago, no further cancellation or re-credit of miles.
Well I did the cancellation by phone and have no cancellation reference or so just the original PNR.

Any experience or help with this ?
