Master then surname was used at uk prep schools which go to age 13. At there public school boys would be just addressed just by surname. If there are 2 brothers at the same prep school the younger one would sometimes be called by Master then first name. We had a very traditional maths teacher at my prep school and he would always use Master.
We did have a kid called Neil Bates, hours of amusement in maths, but the teacher never seemed to get the joke, perhaps he did and just liked that kids life a misery in the playground.
It does seem to have died out in the uk, but we used to live in the Caribbean, and there it still seems common, but only using first name, you would also be addressed by other adults, as Mr/Miss first name, all a bit weird.
