The Mastercard promotion rate should work provided you don't confuse the issue at check-in. Be sure to tell them you want to earn American Airlines AAdvantage airline miles as opposed to Starpoints and stay credit to your SPG account.

I truly do not know about the Kamaina rate. Does it normally earn Starpoints? If so, it will also earn airline miles.

As far as the 5,000 airline mile bonus promotion is concerned, the reservation does not have to be booked online at Starwood's proprietary web sites. It just has to be an eligible rate to earn airline miles.
