Not always. For a while non-elites couldn't upgrade at all on cheaper domestic fares. Then they came up with the co-pay model instead. The co-pay has been around for a few years now and it was probably 2 years of no upgrades for non-elites on cheaper fares prior to that, though I'm having trouble finding the exact dates.
