It's a picture card with the name of the hospital and then something like this:

JOHN DOE, MD

He told me no one ever demands a state ID instead.

What would an FA say?
Back when I worked at a big hospital, I was swapping travel stories with a MD on staff and he also ran into an in-flight emergency (fracture which was just stabilized and didn't even divert the flight). The FAs accepted his hospital ID and business card as sufficient.
