Experiences with calling ahead for room upgrade?

For those veteran Gold or Platinum members that generally call ahead before their reservation to ask for a room upgrade, what are your experiences? Specifically:

1- How many days in advance do you call?
2- Who do you ask to speak with?
3- Have you found this helps with upgrades, or is it a waste of everyone's time?
