Not a bad idea here. I think I'd go with "Misconduct." "Failures" and "Incompetence" carry too much risk of implying "here's where they weren't good enough at their jobs, so train them more." "Misconduct" implies that "These people are acting in a manner unbecoming of a public servant. Discipline and reprimand them, or terminate them from their jobs."
