It varies by hotel, but usually a confirmation email will be sent that somehow indicates a suite upgrade. Sometimes it`s noted at the bottom of the confirmation email, sometimes the reservation will show as being for one room and one suite, etc...

If for whatever reason it is not clear to me that the upgrade has been confirmed by the hotel, I usually call the property to make sure they have me booked in a suite.
